Findings: Facility self reported that on July 7th in the afternoon a child in care accidentally received two doses back to back of his medication by two separate staff. Both staff were interviewed and stated that the child had received the medication from one staff at 3:00pm and since the IPAD alarm had gone off the second staff person did not check the medication log and gave the medication again before realizing once she was logging it that it had already been given. The front office staff was immediately made aware and child's guardians were contacted as well as the staff reached out to Poison Control and MSDE Nurse Consultant.

Findings: The center self reported a situation yesterday where two school aged children had the same medication prescribed and both children take the dosage at noon. The Teacher interviewed stated that both children's names were similar and she got confused and gave one child the other child's medication which was the same however a different amount. Parents were contacted as well as OCC, Poison Control and the MSDE Nurse Consultant. The child's physician was also contacted and was not concerned since the difference in the dosage was small. An Medication Error Incident Report Form was also submitted. The child was monitored for any symptoms throughout the day and seemed okay. The center has corrected the situation by having two separate Teachers each with Medication Administration Training give them their medication separately.
