Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 3/13/2026 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. 2 of 2 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Emergency Disaster Plan update form was completed at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation link to the Provider. ****Please email the following as soon as they are obtained: 1. Current Fire inspection. 2. The dimension of the 2 playgrounds. The following was discussed and not limited to: 1. Staff files must be complete. 2. Moving the climbing equipment from the wall of a busy street. 3. Diapering must be done in an approved diapering room. 4. Toilet room ceiling vents must be maintained in a clean condition. 5. Keeping the lid on the soiled diaper container. 6. Using the CBC portal for submittal results. 7. Outlets must be covered in rooms used by enrolled children (the room was not being used at the time.) 8. Updating the written accident, evacuation and emergency plan with current information. 9. Maintains current documentation on facility premises (insurance). 10. Meal components and specific items on the menu. 11. The exposed landscape fabric in the wood chips. 12. Extension cords must not be used. 13. Outdoor toys and equipment must be maintained in a clean condition.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 3/18/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to a staff in charge at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan update form was emailed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. The unsealed edges on the Toddler table. 2. TB tests must be placed in staff files. 3. The low hanging oleanders on the Toddler playground. 4. Moving play structures away from the fence near the busy street. 5. Windows located within 36" from the floor and accessible to children must be manufactured, fabricated, or treated to prevent the glass from shattering or flying. 6. Mixing enrolled and non-enrolled children. 7. The name of the designee on the main posting board. 8. The statement of services must include all 18 required items. 9. The written accident, evacuation and emergency plan must be updated every 12 months. 10. Detailed items on the lesson plan. 11. Unused outlets must be covered with a safety plug or insert.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 4/32024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but not limited to: *The difference between staff meetings versus what counts towards annual training hours. For training that is completed as part of a staff meeting, only the time period of the actual training can count towards the staff's required annual training hours, rather than the entire staff meeting counting towards training hours. In addition, the training would count if it meets one of the topic areas listed in R9-5-403.B. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.
