| Findings: Supervision Concerns. There were concerns that due to a lack of active supervision, a child was hit repeatedly, had their hair pulled, and had their shirt pulled up over their head. The health department investigated this complaint and watched video that confirmed what was alleged in the referral. Staff was not actively supervising the entire classroom which led to delayed intervention and prolonged abuse of the child. Resolution: This provider held a meeting with the staff members involved and gave a verbal warning. IdahoSTARS is working with this provider to hold an Active Supervision TTA for the staff. Additionally, it was reported that the provider terminated the lead teacher and gave the assistant teacher a warning. | |||

| Findings: Supervision Concern. There were concerns regarding unattended children in a bathroom. At time of investigation, it was confirmed that two children were in the restroom unsupervised by staff for an extended period of time. Resolution: As a result of this incident, the facility now keeps t he restroom door closed and an alarm has been installed when a child enters. This will alert staff and ensure proper supervision. | |||
