Noncompliance Area: On 4/4/2024 certification representative viewed video footage of an incident that occurred on 3/13/2024. It was observed at the 4:49 38 pm second mark on the video Child #1 was sitting on the floor of the cafeteria/gym beside the side door used by parents to pick up their children who attend the SACC program. Staff Person #1 is observed to raise their arm and strike Child #1 on the head with their hand. The child was observed to grab their head with both hands and get up and run out of the cafeteria/gym via a set of double doors leading to a hallway in the school. The child is observed to be crouching behind the door in the hallway. At the 4:50 32 pm second mark of the video Staff Person #1 is observed to grab the child by the arm and drag the child into cafeteria/gym. Staff Person #1 is observed to close the double doors to the hallway after releasing Child #1's arm.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child. Staff Person #1 is required to complete the following trainings via the Better Kid Care website: "Building Relationships with Children and Youth Who Challenge Us" and "Tools of the Trade for School-Age Practitioners: Positive Guidance." The Director of Child Development, Staff Person #2 shall make random visits to the location Staff Person #1 is working at a minimum of once a week to observe that positive guidance techniques are being carried out, record the observations on a document approved by Central Region Bureau of Certification and provide support for improvement as needed. The forms must be provided to the Central Region Office on a weekly basis for the remainder of the 2023-2024 school year.

Staff Person #1 was removed from the location and suspended from employment pending completion of necessary trainings. Staff person #1 completed 8 hours of assigned trainings and one week of on-site training and observation before returning to normal work hours. Staff person #1 was removed from desired location for the remainder of the school year. Staff Person #1 will be monitored for the remainder of the school year. Weekly observations focusing on positive guidance techniques will be conducted and reported each week to DHS by Staff person #2.

Staff were given name cards in place of pictures while their camera was not on site. Names cards were distributed to each staff member in accordance with their emergency groupings. Photos of new students will be taken to take the place of these name cards once able. Staff were reminded that they are responsible for knowing who is in their group and their location at all times

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. The required staff:child ratios must be maintained at all times.

Implementing ratios based on our youngest child, staff will maintain a 1:12 ratio at all times. Staff will teach children ratio amounts and utilize a system for grouping students quickly in order to answer doors or use restrooms. Child count will be prioritized to ensure the correct number of children are with staff at all times.
