| Findings: A complaint was received that a staff person "popped" a child on the leg because she would not sit for circle time. Shortly after, the owner/director contacted this Licensing Specialist to report that yesterday they were informed by a staff person that on Monday one of the teacher's in the Tiny 2's classroom handled the children in manner they described as rough. The owner/director reviewed the video tape from the classroom. She reported that the video showed the staff person pick some of the children up and place each down hard on their bottoms on the floor. The staff person was terminated yesterday evening. The Licensing Specialist reviewed the tape and observed the staff person, in a span of 2 minutes, pick 4 different children up from one spot and place them back on their bottoms on the floor in an aggressive manor. In addition, one child was laying on the floor and the staff used her hand and hit the child on her leg two times and then dragged her closer to her so she could wipe her nose. She then picked her up and aggressively placed her on her bottom on the floor. All of this occurred when she was trying to have the children sit in a structured manner for circle time. | |||

| Findings: The outdoor play space has 2 large areas where the ground is sunken that pose a tripping hazard, there are 2 places along two of the landings that lead out from the building where the lattice is coming away from the decking, there is an area on the back fence that the fencing is coming apart from the metal cross bar and the gate on the right has a gap where the two gates meet that is big enough for a child to pass through. The Licensing Specialist walked the outdoor space with the director and explained each of these issues with her. She was picked up by the staff person because of the weather and the fact that school had an early dismissal. | Findings: When the Licensing Specialist arrived there were 13 children in Classroom 1, the 2's room. There were 4 infants, 3 toddlers and 6 two year old children. There were a total of 9 staff in the classroom, 2 infant/toddler teachers, 2 preschool teachers and 5 aides. Due to the heat not working in Rooms 4A, 4B (infant and toddlers room) and Room 3 (2's room) they combined those children with the other 2's classroom children. | Findings: When the Licensing Specialist (LS) arrived there were 25 children present in Classroom 2, Preschool 4's. Because the heat was not working in Classroom 5, Preschool 3's, the two classes were combined. The LS observed two children sitting at a table in the back of the classroom, one appeared to be middle school age and the other elementary age. The LS asked staff, M.V., who the older child was, she hesitated and asked that I speak to C.R. about it. She said the younger child was her daughter and that she was enrolled but again directed the LS to speak to C.R.. As we were talking the LS observed the older child walk out of the classroom. The LS asked C.R. about the child and she said that she was there to pick up a key from her mother, V.L., that worked there and they would be leaving. Shortly after this the LS went to the office, the child was sitting in a chair and V.L. was talking to her. V.L. identified the child as her 13 year old daughter. She said that due to the snowy weather and the schools early dismissal she picked her up from her babysitter and brought her to the center. | |||

| Findings: The complaint was that the lunch food is prepared in the morning so by the time its lunch time the staff have to reheat the food and by the time it gets delivered to the classroom it is cold. According to management and staff that were interviewed, the cook comes in the center prior to opening to begin to prepare breakfast and lunch. He reportedly is usually gone by 8:30am. The prepared food is stored on top of the stove in metal trays and covered with plastic and aluminon foil. At lunch time designated staff will put the prepared food onto individual plates and deliver to the classrooms. The children in the preschool and 2's classrooms will all eat when the food is delivered. The infants and toddlers that eat the lunch food may or may not eat at the time the food is delivered, it depends on if they are napping or not. The staff will not wake the children up to eat so the food may stay in the classroom for a short time until the child wakes up. When this happens the staff will reheat the food up in the microwave. The center can not say for certain that the food is maintained at 140 degrees F before serving. | |||

| Findings: The complaint alleged that staff do not use the handwashing sink in the kitchen for handwashing but rather use the 3 bay sink to wash their hands. The handwashing sink did not turn on when the LS put her hand under the sensor. During the follow up visit the owner, D.T., explained that the sensor on the handwashing sink is not working correctly (the water will not stop running) so they turned the water off to the sink.
