2026-02-24 According to staff statements and video footage, 3 children were not within sight and sound supervision to prevent harm and ensure safety for approximately 10 minutes on 2/13/2026. According to staff statements, 3 children in the Yellow Room had left their assigned group, and travelled down the hallway to the bathroom, where they were found playing in the toilet. One of those 3 children escaped a second time and was found by himself in the center kitchen. It was observed on the center's video footage that the 3rd child was returned to the classroom several minutes after the first 2 children. Violation

The children were noticed playing in the toilet in the hall bathroom by a staff member from a classroom across the hallway, and an additional staff member in the center collected and returned the children to the Yellow Room. After the 3 children were returned to their classroom, one of the 3 children escaped again and was found in the center kitchen, according to staff statements. Violation

It was observed during the complaint visit that the toilets contained toilet bowl cleaning product, resulting in children potentially ingesting poisonous chemicals at the time of the incident.
