LACKING REQUIRED HIRING DOCUMENTS: Facility Person #5 -- May not work in a child care position at the facility until completed child abuse clearance are obtained. A facility's person record shall include a copy of the completed clearance information required under CPSL.

Noncompliance Area: At the time of the inspection, two ceiling tiles in room 6 and three ceiling tiles in room 9 were observed discolored.

Noncompliance Area: At the time of the inspection, visual identification or sticker decals were not observed on the glass doors in all child care spaces. Non-compliance was corrected on site.
