Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: A renewal inspection was conducted on 6/2/2026. At that time, an inhaler for Child #6 was not in the original container.

Correction Required: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received.

Noncompliance Area: A renewal inspection was conducted on 6/2/2026. At that time, Child #4 was observed being fed while sitting in a Boppy pillow (see LIS code sheet for DOB).

Correction Required: An infant 6 months of age or younger shall be held while being bottle fed.

Noncompliance Area: A renewal inspection was conducted on 6/2/2026. At that time, staff files were reviewed. Most recent PSP and Child Abuse clearances on file for Staff Person #1 are both dated 5/24/2021, which is past the 60-month requirement.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #1 may not work in a childcare position with direct contact and routine interaction with children until all completed clearances are on file at the facility.

Noncompliance Area: A renewal inspection was conducted on 6/2/2026. At that time, an uncovered outlet was observed in the Safari Room.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Description: Contaminated trash in closed plastic-lined receptacle

Noncompliance Area: A renewal inspection was conducted on 6/2/2026. At that time, uncovered trash cans containing tissues were observed in the Pre-K and Rainbow Rooms.

Correction Required: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles.

Description: Infant sleep position

Noncompliance Area: An allocated unannounced monitoring was conducted on 1/20/2026. At that time, an 8-month-old infant was observed sleeping in a car seat placed inside a crib. A second infant was observed sleeping in an infant seat.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. TIERED LIS: 1. If any of the staff have already completed this training, another training related to infant safe sleep may be substituted with Northeast regional Office approval. The operator shall provide a date in which the training will be completed.

Description: Written emergency plan posted

Noncompliance Area: An allocated unannounced monitoring visit was conducted on 1/20/2026. At that time, the emergency transportation plan was not posted in any measured child care spaces.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Description: Pertinent Laws & Regulations/General Health and Safety

Noncompliance Area: An allocated unannounced monitoring visit was conducted on 1/20/2026. At that time, staff files were reviewed File for staff person #2 contained a mandated reporter training certificate dated 10/6/2024. The certification representative noted that the formatting and appearance of the certificate differed from standard mandated reporter training certificates. The certification representative submitted the certificate to the CPSL Help Desk for verification. The CPSL Help Desk confirmed that the certificate was fraudulent. Staff person #2 does not have valid mandated reporter training. Additionally, the file for Staff Person #2 contained a Heartsaver First Aid CPR AED training certificate dated 10/1/2024. The optional modules listed on the certificate included "Heart Saver Pediatric First Aid with, Child CPR AED, Infant CPR Exam." The certification representative noted that the appearance of the certificate differed from other Heartsaver Pediatric First Aid CPR AED certificates. The certification representative scanned the QR code on the certificate in the file and observed that the optional modules displayed differed from those listed on the certificate. Based on the documentation reviewed, Staff Person #2 did not complete pediatric first aid/CPR training from a PQAS-approved instructor. Staff person #2 was observed to be caring for children alone in the nap room during the inspection.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings are completed, staff person #2 must be supervised, when interacting with children, at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #2, staff person #2 may not work in a child-care position at the facility.

Noncompliance Area: An allocated unannounced monitoring was conducted on 1/20/2026. At that time, staff files were reviewed. The health assessment on file for Staff Person #3 indicated that the individual has a communicable disease; however, the file did not include additional documentation describing the condition or addressing any potential risk to others who may be exposed.

Correction Required: An adult health assessment must include an examination for communicable diseases and the results of that examination.

Description: Emergency plan

Noncompliance Area: An allocated unannounced monitoring was conducted on 1/20/2026. At that time, the facility's emergency plan and emergency plan letter were reviewed. The emergency plan letter, which is provided to parents at enrollment, did not include provisions for accommodations for infants, toddlers, children with disabilities, or children with chronic medical conditions.

Correction Required: The facility shall have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions.

Description: Health and Safety Training

Noncompliance Area: An allocated unannounced monitoring visit was conducted on 1/20/2026. At that time, staff files were reviewed. The file for Staff Person #2 contained a Heartsaver First Aid CPR AED training certificate dated 10/1/2024. The optional modules listed on the certificate included "Heart Saver Pediatric First Aid with, Child CPR AED, Infant CPR Exam." The certification representative noted that the appearance of the certificate differed from other Heartsaver Pediatric First Aid CPR AED certificates. The certification representative scanned the QR code on the certificate in the file and observed that the optional modules displayed differed from those listed on the certificate and did not include pediatric first aid/CPR. Based on the documentation reviewed, Staff Person #2 did not complete pediatric first aid/CPR training from a PQAS-approved instructor within 90 days of their date of hire, as required (see LIS code sheet).

Correction Required: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10 or (g)1-10. Until such time as the required training has been completed, staff person #2 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #2, staff person #2 may not work in a child-care position at the facility.

New staff may not be unsupervised with children until all preservice trainings have been completed and documentation is on file at the facility. The correction date for this portion of the plan shall be a minimum of 3 months from the date the Regional Office receives an acceptable plan.

Regulation: 3270.35(b)(2)/3270.192(2)(iv)

Description: Bachelor's, 30 credits + 1 yrs/Transcript, diploma and letters

Noncompliance Area: An allocated unannounced monitoring visit was conducted on 1/20/2026.At that time, the staff files were reviewed. The file for Staff Person #1 contained a foreign degree that had not been evaluated for equivalency in the United States by a service that is a member of the National Association of Credential Evaluation Services (NACES) or the Association of International Credential Evaluators (AICE).

Correction Required: A group supervisor shall have a bachelor's degree from an accredited college or university, including 30 credit hours in early childhood education, child development, special education, elementary education or the human services field and 1 year of experience with children. A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.
