Noncompliance Area: At the time of the Renewal Inspection conducted on 04/24/2026, Staff Person #1 did not wash Child#1's hands after diapering and before returning Child #1 to the highchair for lunch time.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: At the time of the Renewal Inspection conducted on 04/24/2026, Child #1's Sippy cup was unlabeled on the shelf in the childcare space.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: At the time of the Renewal Inspection conducted on 04/24/2026, the most recent Fire Inspection Report on file was dated 03/05/2025.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met.

Noncompliance Area: During the renewal inspection on 12/19/24, it was observed that the records of child #1, child #2, and child #3 did not include the work addresses and telephone numbers of the enrolling parent.

Correction Required: Emergency contact information must include the home, and work addresses and telephone numbers of the enrolling parent.

Description: Toddler/preschool: updated health report every 12 months

Noncompliance Area: During the renewal inspection on 3/13/24, it was observed that the records of child #1 did not include an updated health report. Health report submitted was dated 2/8/2023.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Regulation: 3270.151(a)/3270.151(c)(2)

Description: 12 months prior to service and every 24 months thereafter/Mantoux TB

Noncompliance Area: During the renewal inspection on 3/13/24, a health record dated 8/23/23 and Tuberculosis screening by Mantoux method dated 8/25/23 was submitted for staff person #2. Upon further review the health record and TB testing submitted was verified as fraudulent.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: During the renewal inspection on 3/13/24, Pediatric first Aid & CPR certificates dated 2/1/2024 respectively, were presented for staff person #1 and staff person #3. Upon further review, the Pediatric First Aid certificates were verified as fraudulent.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Description: Comply with CPSL/CPSL information

Noncompliance Area: During the renewal inspection on 3/13/24, it was observed that the records of staff person #4 did not contain the correct Child Abuse clearance for an AGS. A volunteer child abuse clearance dated 4/12/2022 was submitted for staff person #4.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 4 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
