A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Noncompliance Area: Staff 1, was seen on video, grabbing a child, who was walking away from staff 1, by the hair to get the child to come back to staff 1.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

Noncompliance Area: During the inspection, a child was observed to be sitting behind the half door in the middle room on the right side of the building, out of the sight of the staff, putting their nap items away.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: Staff 1 was seen on video. Staff 1 "popped/flicked" a child's hand that was in her mouth.
