Noncompliance Area: The file for child #3 lacked a current emergency contact form and agreement updated by the parent within the last 6 months. The files for child #1 and #4 had an emergency contact form and agreement that exceeded 6 months between updates by the parent.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: According to the fire drill log, the hypothetical location of the fire was the same for consecutive fire drills.

Description: System in compliance/Operator or designated staff responsibility

Noncompliance Area: During the renewal inspection on 2-3-26, the interconnected smoke detectors were tested. The basement and main floor detectors were observed to be operating as interconnected. The upstairs detector did not make a sound and function as interconnected when the other two detectors were tested and functioning as interconnected. The upstairs detector was tested and worked independently from the other two detectors.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Description: Clean and good repair

Noncompliance Area: In the play room, there was a fabric chair with multiple tears in the fabric.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Locked or inaccessible - in original container

Noncompliance Area: In the bathroom upstairs, there were multiple toxics, including bottles of Lysol and Clorox disinfectant cleaners, on the back of the toilet, on the floor next to the sink, and in an unlocked cabinet that were labeled "Keep out of reach of children" and were accessible to children. On the front porch, there were a bottle of Simple Green Outdoor Odor Eliminator and a Mainstays Torch Fuel Canister that were labeled "Keep out of reach of children" and were accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Description: Clean and good repair/Small Toys and Objects

Noncompliance Area: In the playroom, there was a fabric chair that had multiple tears with small pieces of fabric coming apart that were less than 1 inch in diameter and were accessible to young toddlers who are still placing objects in their mouth and are enrolled at the facility.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Description: Vet's certificate required

Noncompliance Area: A veterinarian's certificate of current rabies immunization was not on file for a dog that was observed at the facility.

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.
