2026-06-04 Other 13A.15.03.02A Corrected
Findings: There is a 2 year old who is enrolled that provider did not mention when specialist was at the home last week. She states she doesn't have the file ready to show to specialist. Provider needs to send copies of the child's record to specialist ASAP. Provider is reminded to have all children's paperwork in place prior to starting them in care.
2026-06-04 Other 13A.15.08.01B(4) Corrected
Findings: When specialist arrived the provider was up on the main level of the home and the children were in the family room which is 1/2 a level lower than the main level of the home. She was not visible to the specialist. The specialist called out to her in the doorway prior to entering the home as a child had opened the door to the specialist. Provider then appeared at the top of the stairs to the main level. She stated she had gone up there to be sure her husband (approved substitute) had heard her when she called to him to come to take the children out to the bus stop. Provider is reminded that she must remain on the same level of the home as the children because when she is on another level she is not near enough to render immediate assistance to the children. She returned to the family room level which corrected this.
