Noncompliance Area: At approximately 1245pm it was observed that 4 cots and 1 pack and play were in use and did not have at least 2 ft of space on 3 sides.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Description: Refrigerator

Noncompliance Area: The refrigerator did not contain a thermometer.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Description: Infant sleep position

Noncompliance Area: At approximately 1135am it was observed that Child #1(See LIS Code sheet) was placed in a swing to sleep which is not the sleeping position recommended by the American Academy of Pediatrics.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Noncompliance Area: Child #2's two most recent child service reports are dated 6/2025 and 1/2026 which are more than 6 months apart.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e).

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Correction Required: The operator shall require the parent of an enrolled child to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record must contain initial and subsequent health reports.

Noncompliance Area: The immunization record on file for Child #1, an infant age child(See LIS Code Sheet), includes 0 of 1 DTAP vaccines recommended by the ACIP for a child this age; 0 of 1 HIB vaccines recommended by the ACIP for a child this age; 0 of 1 Pneumococcal vaccines recommended by the ACIP for a child this age; and 0 of 1 Polio vaccines recommended by the ACIP for a child this age.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Description: Surfaces cleaned

Noncompliance Area: At approximately 1130am it was observed that the diaper changing surface was only wiped with a baby wipe and not a sanitizing solution between changes.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Description: Locked or inaccessible - in original container

Noncompliance Area: In the kitchen on a low shelf near the door way accessible to children was Purell Hand Sanitizer and Hemz Candy Cane Lane Lotion both stated keep out of children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The fire alarm testing log contained the following dates 1/2/26-2/2/26 and 3/2/26-4/2/26 which at more than 30 days apart.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Description: Supervised at all times

Noncompliance Area: At approximately 10am on 6/10/25, Staff #1 placed a young toddler age child on the diapering table and walked away to gather supplies leaving the child alone unsupervised on the diaper table.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3290.51-3290.52 (relating to maximum number of children; and staff:child ratio).
