2025-09-03 According to the center policy, diaper changes will be provided as a needed basis. During interviews with staff and the center director, a childs diaper is to be changed every two hours or every hour if a child has a rash and cream is applied. On 7/22/25, a child started to obtain a rash. Based on the childs transition report, a diaper change was conducted on 3:36pm, at which time, cream was applied. Staff failed to change the childs diaper one hour later. Violation
