Our experts designed our classrooms - and every activity and lesson - to help prepare your child for success in school and beyond. With designated learning centers such as dramatic play and blocks in every classroom, children have the opportunity for rich social play and child-initiated discovery.

Staff Person #1 (DOH see LIS code sheet) and Staff Person #2 (DOH see LIS code sheet) stated that Staff Person #1 was standing at the end of the hallway closest to the preschool room while Staff Person #2 was standing at the other end of the hallway by the playground calling children's names and having them line up in the hallway as Staff Person #1 checked off the children's names on the CSR form. Staff Person #2 stated that once all the children were inside Staff Person #2 did a visual sweep of the playground and closed the door while Staff Person #1 counted 20 children and they all proceeded to walk into the classroom. Staff Person #1 and Staff Person #2 stated that they never noticed Child #1 leave the group and go back out to the playground. Staff Person #3 observed Child #1 standing outside the window of the DPS A classroom. When Staff Person #3 realized Child #1 was alone on the playground they notified Staff Person #4 and #5. Staff Person #4 and #5 immediately retrieved Child #1 from outside on the playground and returned Child #1 to the preschool room. Staff Person #1 and #2 did not know the whereabouts of Child #1 for approximately 5 minutes. Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. Toilet water spilled onto the floors in the affected classrooms and into the hallway near the staff bathroom, seeping into the DPS A room under the cubbies mounted to the wall and into a storage area by the kitchen. Staff Person #1 instructed Staff Person #2 to put a pull up and/or diaper on potty trained children if they had to go to the bathroom while they were waiting for the toilets to be fixed. Staff Person #2 and # 3 reported that multiple children were crying and upset because they didn't want to wear a pull-up or diaper. This act did not promote the development of skills, social competence and self-esteem. In all classrooms we will ensure we are promoting the development of skills, social competence and self-esteem by ensuring the children have proper access to the toilet or diapers/pull-ups depending on what is needed for the individual child. A child who is potty trained and no longer wears diapers and/or pulls will not put a child in diapers and/or pull ups against their will or without the direction of their parent or guardian. If bathrooms are inaccessible we will not allow children to remain in care and must be picked up immediately. |
