Noncompliance Area: During the complaint investigation conducted 6/10/26, there were 16 children (the youngest child in the group was an infant age child) present in the infant classroom with staff person # 1, # 2, # 3, # 4, and # 5, which exceeds the maximum group size for an infant.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30.

Noncompliance Area: During the unannounced inspection conducted 7/8/25, three infants were observed sleeping in cribs inside of the infant room childcare space without having at least 2 feet of space between each crib. Four cribs were observed close together in a row with three out of the four cribs being occupied by sleeping infants.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Description: Inpatient hospitalization or ER treatment of child

Noncompliance Area: During the complaint investigation conducted 7/8/25, it was found that child #1 was seen at an urgent care physician's office to receive medical treatment for an injury that occurred at the facility during a fire evacuation. The operator did not telephone notice the appropriate regional office within 24 hours.

Correction Required: The operator shall immediately notify a child's parent and shall telephone notice to the appropriate regional office within 24 hours if a child in care at the facility is hospitalized or receives emergency room treatment.

Description: General Health and Safety

Noncompliance Area: During the complaint investigation conducted 7/8/25, it was found that child #1 was seen at an urgent care physician's office to receive medical treatment to the foot for an injury that occurred at the facility during a fire evacuation.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: During the complaint investigation conducted 5/7/25, it was found that staff person #1 was observed aggressively approaching staff person #2 and communicating in a harsh manner, in the presence of 5 pre-k children and one young toddler.

Correction Required: A facility person may not use harsh, demeaning or abusive language in the presence of children.

Description: Supervised at all times

Noncompliance Area: During the complaint investigation conducted 4/8/25, it was found that staff are often on cell phones and not supervising children.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Description: Restraints prohibited

Noncompliance Area: During the complaint investigation conducted 4/8/25, it was found that, children are often strapped down in child size chairs with t-straps for extended times after am snack, lunch and pm snack.

Correction Required: A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.

Description: Towels labeled and laundered

Noncompliance Area: During the complaint investigation conducted 4/8/25, It was found that rags for cleaning surfaces and cleaning children were being used without being labeled. Cloth rags that are used for cleaning surfaces are also being used to wash children's hands and nose. The same rags are used for multiple purposes and are not labeled for use of specific children.

Correction Required: Cloth towels and washcloths shall be labeled with the child's name, used by only the named child and laundered weekly. The director shall arrange a laundry schedule with the parent.

Regulation: 3270.153

Description: Facility Person/Symptoms of Disease

Noncompliance Area: During the complaint investigation conducted 4/8/25, It was found that staff have been allowed to work while sick with a contagious illness while the facility was made aware.

Correction Required: A facility person with symptoms of a communicable disease or infection that can be transmitted directly or indirectly and which may threaten the health of children in care shall be excluded from attendance until the facility operator receives notification from a physician or CRNP that the person is no longer considered a threat to the health of others. The notification shall be retained in the facility person's file. Exclusion from the facility is required for diseases and conditions specified in 28 Pa. Code Chapter 27 (relating to communicable and noncommunicable diseases).

Noncompliance Area: During the unannounced inspection conducted 4/8/2025, the third-floor school age childcare space was being utilized as a storage space. There were tools, boxes, shelves, large cushions, extra toys and a vacuum cleaner being stored.

Correction Required: A facility shall provide indoor child care space for individual and group small muscle activity.

Description: Locked or inaccessible

Noncompliance Area: During the unannounced inspection conducted 4/8/2025, pot and pan sanitizer, re-Juv-Nal disinfect, and top clean soap were observed on a low shelf in the school age 3rd floor bathroom.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Description: Stairs, exits, etc. unobstructed

Noncompliance Area: During the unannounced inspection conducted 4/8/2025, the back hall space exit, before entering the preschool room was observed obstructed with boxes, plastic storage containers, a table, chairs, tarp and a garden wheelbarrow.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.
