Noncompliance Area: At approximately 11:51 AM on 6/11/26, Staff #1 and Staff #2 were observed in the Main Room with a total of 10 children. Staff #1 named 1 Older Toddler & 3 Preschoolers, Staff #2 named 4 Preschoolers, and 2 children were not named in a supervision group. During a second attempt, Staff #1 named 1 Older Toddler and 3 Preschoolers and Staff #2 named 4 preschoolers in their supervision groups, leaving 1 Preschool child unaccounted for.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: 161c A lunchbox containing potentially hazardous foods from home was observed on the front desk and was not refrigerated.

Correction Required: Potentially hazardous food brought from the child's home or provided by the facility shall be refrigerated.

Noncompliance Area: SECOND CITATION: Peeling paint and exposed rust was observed on the exterior of the doorframe in the Outdoor Play Space and was accessible to children. PREVIOUSLY CITED ON 5/14/26.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Description: FIRE DETECTION/Staff responsibility

Noncompliance Area: SECOND CITATION: The most recent documented fire alarm testing was dated 4/2/26, more than 30 days prior. PREVIOUSLY CITED ON 5/14/26.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. §  1016(c)). The primary staff person or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met

Regulation: 3280.102(a)

Description: Clean and good repair

Noncompliance Area: A toy riding horse with a broken suspension strap, a tool bench with a broken leg, and a detached board on the back of the steps on the play structure were observed to be accessible to children in the Main Room and not in good repair.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Small Toys and Objects

Noncompliance Area: A chair with torn foam was observed in the Outdoor Play Space and was accessible to children who are still placing items in their mouths.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The initial child service reports on file for Child #4 and Child #5 are dated 8/8/25, more than 6 months from the child's date of enrollment (SEE IS CODE SHEET).

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3280.124(f).

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care. A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Noncompliance Area: The most recent parent review and signature for the emergency contact form on file for Child #1 is dated 8/7/25, more than 6 months prior. The only parent review and signatures for the emergency contact forms on file for Child #4 and Child #5 are dated 12/5/25, more than 6 months from the child's date of enrollment (SEE IS CODE SHEET).

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Description: Health Information/Initial and subsequent health assessments

Noncompliance Area: The most recent health assessments on file for Child #1 and Child #3 are dated 2/12/25, more than 12 months prior.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record must contain initial and subsequent health reports.

Noncompliance Area: The two most recent health assessments on file for Child #4 (6/11/23 & 6/11/25) and Child #5 (5/10/23 and 6/11/25) are dated more than 12 months apart.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record must contain initial and subsequent health reports.

Description: Immunization record/Dismissal policy

Noncompliance Area: The immunization record for Child #2 (PSA) lacked the following immunizations: 3 doses of DTAP, 1 dose of Pneumococcal, 3 doses of Polio, and 1 dose of Hepatitis A without a Letter of Exemption in the file. The immunization record for Child #4 (PSA) lacked the following immunizations: 1 dose of Rotavirus, 1 dose of DTAP, 1 dose of Pneumococcal, and 2 doses of Hepatitis A without a Letter of Exemption in the file. The immunization record for Child #5 (PSA) lacked the following immunizations: 1 dose of HiB and 1 dose of Pneumococcal without a Letter of Exemption in the file.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings).Child #2, Child #4, and Child #5 must be dismissed from care by close of business on 5/14/26 and may only return to care when the immunization record is updated or when a parent provides a written, signed exemption letter.

Noncompliance Area: At the time of inspection, it was observed that a group of toddlers did not wash their hands prior to eating snack.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: The facility's Shaken Baby Syndrome/Abusive Head Trauma policy lacked details regarding the prevention and identification of child maltreatment.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.
