| Findings: Supervision. At time of investigation, it was reported that a staff members walked a child to their elementary school where they dropped the child off with a teacher there. However, the child was dropped off for the wrong class time and they were not aware of their mistake until the school called roughly 90 mins later. Facilities phone was not working at time of the incident, so a call may have been received earlier. Resolution: The provider is taking additional measures to increase safety at the facility. Now during transport, the children are given name tags to wear before they leave. The children are signed out at the facility when they leave, and signed off again when dropped at the location. The staff members have also been instructed to clearly communicate with the school staff during drop off. | |||
