| Findings: LS conducted a complaint investigation on today's date. The complaint states that during outside playtime, a male child was playing on a part of the playground where children should not be. He was playing on that part of the playground and was redirected a few times before Savannah held the male child by his legs, causing him to hit his head and back on the playground. LS observed video footage of the incident that occurred on 4/9/2026. LS observed the child referenced in the incident attempting to climb through an opening not intended to be crawled through on the play structure. LS observed an aide in room 8 go over to the child and grab the child's legs in an attempt to catch the child. LS observed the child hit their head on the play structure after the aide grabbed their legs. The child then hits their head again on the turf ground. The child is then observed holding their head, and the aide is observed immediately walking away from the child without checking on any injuries, even though the child appears to have hurt their head. The child then runs over to another teacher to be comforted. The operator shall ensure the safety and security of each child at all times. | |||
