Findings: The OCC received a self-reported incident stating that (3) children ran around a corner and into an alcove near the PEP playground. This area was not in direct view of the staff members supervising the children. During this time a child exposed their underwear at the direction of another child. There were (2) staff members already on the playground with another small group of children and approximately (8) other staff members and a group of approximately (80) children heading out to the playground when the incident occurred.
