Noncompliance Area: Facility staff #3 did not have documentation of a health assessment within 12 months prior to providing initial service to children on file at the time of inspection.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: At the time of inspection on 9/17/2024, Certification Representative observed facility staff #1, #2, #3 and #4 working with children and there was not the proper documentation that all clearances were requested prior to employment. Facility staff #1 had a hire date of 8/29/2024. The disclosure statement on file for facility staff #1 was dated 8/7/2024. The FBI was not completed until 9/14/2024 and the NSOR request was completed 8/30/2024. Facility staff #2 had a hire date of 9/3/2024. The disclosure statement was signed 8/30/202 The FBI was not completed until 9/13/2024 and the NSOR request was completed 9/4/2024. Facility staff #3 had a hire date of 9/15/2024. The disclosure statement was dated 9/6/2024. The NSOR request was completed 9/12/2024. Facility staff #4 had a hire date of 9/3/2024. The disclosure statement was signed 8/27/2024. The NSOR request was completed 9/4/2024. Facility staff #4 required out-of-state clearances and there was not documentation those requests were made at the time of inspection.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 - #4 may not work in a child care position at the facility until all clearances are obtained.

Noncompliance Area: At the time of inspection on 9/28/2023, Certification Representative observed that facility staff #1 was alone with children and did not have all required clearances on file. The record showed that facility staff #1 lived in NJ within the past 5 years and there was not out-of-state clearances on file. Facility staff #1 was observed to have their own group of children and during a transition time left a classroom with the group and was not supervised by another qualifying staff person.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Description: Safe routes posted

Noncompliance Area: During the renewal inspection conducted on 9/16/2021 observed the safe routes were not posted at a conspicuous location in the facility.

Correction Required: Written notification of safe routes shall be posted by the operator at a conspicuous location in the child care facility.
