Corrective Action: Ensure safe sleep procedures are always followed. Upon arrival, two infants were observed asleep. One was asleep in a highchair and one was asleep on the floor covered up with a blanket on his stomach. This was corrected at the visit when the infants were moved to their cribs.

Corrective Action: A staff member admittedly sent a child to school without first changing her dirty diaper. Ensure children with soiled diapers and/or clothing are changed when the provider becomes aware the child needs changing. The center must provide appropriate diapers and clothing for all age groups being served. This was corrected when the specialists and the administrator discussed the issue and the administrator agreed to comply.
