Findings: LS observed two staff members who were not on the associated parties list and were unknown to the OCC. LS advised staff to send all required documents to be approved by the OCC.

Findings: LS conducted a complaint investigation on today's date. The Center stated ton 10-21-2024 they saw a Teacher pop a 3-year-old child on the hand after saying, quote, you hit me so I'll hit you. Teacher stated that she was trying to get the child to get rid of the child's gum. The child was immediately removed from the room, Teacher was removed and replaced. The child was visibly angry, and kicking the Director. The Director and Center's Family Advocate Supervisor spoke to the child until the child was calm. The child is still enrolled, and has no observable injuries. The child's parents were called and the Teacher was immediately terminated. During the investigation, LS interviewed the Program Director, the Teacher who witnessed the incident, the Center's administrator, and the Center's Coach. All parties stated that the teacher scooped gum from the child's mouth after the child refused to spit it out, and when the child kicked her as a result, she tapped him on the hand and said don't hit. All parties described the child calming himself down in another area. to the class, and returning to circle time on his own. LS discussed that no child should be subjected to abuse or injury.

Specialist was informed by Director that on April 30, 2024 that a 3 year old child was left in the restroom alone for approximately 20 minutes, 9:45am - 10:05am. All centers shall maintain procedures to ensure the whereabouts of all children in known at all times. Corrected onsite.
