Noncompliance Area: Medication for child 1 was at the facility. The medication did not have the child's name on it and the prescription information was not included with the medication. This was corrected on 9/10/25.

Correction Required: A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable. The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: An updated health assessment was not on file for child 4. The health assessment on file for child 4 was conducted on 8/25/23.

Description: Comply with CPSL/CPSL information

Noncompliance Area: Documentation of the state police clearance, child abuse clearance, FBI clearance and NSOR clearance on file for the Facility person #1 and facility person 2 were expired. Facility Person #1's state police clearance expired on 2/15/25, the child abuse clearance expired on 2/15/25, the FBI clearance expired on 3/12/25 and the NSOR clearance expired on 2/25/25. Facility person #2 state police clearance expired on 2/15/25, child abuse clearance expired on 2/27/25, FBI clearance expired 3/9/25 and NSOR clearance expired on 2/25/25.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 --- may not work in a child care position at the facility. Facility person #3 cannot be in the home during childcare hours until all updated clearances have been received.

Noncompliance Area: Documentation of the fire drill log on file showed that fire drills were not being completed every 60 days. Fire drills were conducted on 3/28/24 then again on 5/28/24; which is 61 days, 7/23/24 then again on 9/24/24; which is 63 days, 11/11/24 then again on 1/22/25; which 72 days.

Correction Required: The operator or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that fire drills are conducted at least once every 60 days.

Tested 9/3/21 and again on 10/4/21, which is greater than 30 days. Tested 10/4/21 and again on 11/11/21.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c))The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in § 3290.95(a) are met.
