| Findings: Supervision and Licensing Concerns. There were concerns that a provider at this facility did not have proper background check clearances, and the facility had been operating without a current license. Resolution: The health department performed an investigation and determined the facility was operating without current license, and a new provider was working without proper clearances. Owner was educated that they could watch maximum of 6 children until their license is renewed, and staff could not be on site until they have received proper clearances. | |||

| Findings: Health and Safety Concerns. There were concerns staff were regularly smoking in outdoor areas where children are playing and are not washing their hands when resuming child care responsibilities. The health department performed an investigation and substantiated due to two employees who regularly smoke in outdoor areas during operating hours. Resolution: Health department informed provider that employees cannot be smoking near the kids, need to move further away from facility, and should wash their hands prior to resuming child care responsibilities. Additionally, they recommended staff wear outer garments to put on and remove after smoking. | |||
