2025-05-08 Complaint 13A.15.08.01D(1)(a) Corrected
Findings: LS found 5 pack and plays upstairs. The provider was on the main level with 5 children one child was upstairs crying. The provider has a monitor that has sound however at one point the provider left the 5 children downstairs and went upstairs to get the crying child. She left the 5 children for approximately 2 minutes. Additionally, the provider left the child care area and shut the door behind her leaving 6 children unattended to tend to her dog. She was in the closed room for approximately 30 seconds.
2025-05-08 Complaint 13A.15.09.01A Corrected
Findings: The providers daily schedule was not being followed. I recommend to work with Child Resource connect to achieve a workable daily schedule. The children were all in highchairs upon our arrival at 9:30 some children were eating. Some children had nothing and were given a stuffed animal after our arrival. By 10:25 four children were back in the highchairs 3 with a snack. Children should not be confined to a highchair.
2025-05-08 Complaint 13A.15.09.02A(1) Corrected
Findings: Children two and older were not afforded the opportunity to sit at the table and were placed in a high chair which can be seen as restrictive and is not developmentally appropriate.

2025-03-11 Full 13A.15.08.01D(2)(b) Corrected
Findings: LS asked how the 4 children napping upstairs are monitored and only 1 room has a video access. LS did not see the children monitored every 15 minutes.
