The photograph referenced in this citation was not an official photo of Building Blocks Learning Center, contained no time stamp, and lacks verified context of when or where it was taken. Its authenticity and the circumstances surrounding it remain unclear, and therefore, its interpretation is subjective and inconclusive. Our internal investigation determined that the staff member was not using a child's bedding at any time. The cot shown in the image was used only as seating during rest time and not for the purpose of resting with or under a child's personal blanket. Building Blocks Learning Center maintains a Children's Rest Equipment and Bedding Policy which requires all rest equipment and bedding to be labeled and used exclusively by the assigned child. This policy has been in place and continues to be enforced daily. All staff members reviewed the rest equipment & bedding policy which requires all rest equipment and bedding to be labeled and used exclusively by the assigned child. Staff members are not permitted to utilize any rest equipment or bedding and will follow and uphold this policy. |

During interviews, Rep was provided with a picture of Staff# 1 laying on a cot and facing the wall. Rep also took photos of the entire Primary A room in which this incident occurred. From the view of the room and position in which Staff #1 was in the picture provided, she would not have been able to accurately supervise the remainder of the children since she was facing the wall. Interviews with staff and director indicate she would have been the only staff in the room since this occurred during nap time. Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. The picture in question was not taken by our organization, does not include a time stamp, and therefore cannot serve as complete or verified evidence of the classroom environment at that time. A still photo does not contain sufficient evidence to determine what was occurring across the classroom, nor does it establish duration, context, or teacher engagement. Our internal investigation determined that the teacher was actively supervising children during this period and that no lapse in supervision occurred. Building Blocks Learning Center's Child Supervision Policy, which was reviewed and verified as active at the time of this investigation, requires that staff be able to see, hear, direct, and assess children at all times, including during nap periods. An update to the current supervision policy was created and will be reviewed, signed & dated by all staff. Children will be supervised at all times. |
