During the investigation, it was confirmed that a child had been left outside unsupervised for approximately 20 minutes. | Findings: Supervision Concerns. There were concerns that a child had escaped from the facility. During investigation, the health department and law enforcement both determined that a child had left the facility and was walking out on a busy street for a substantial amount of time. Resolution: Provider has called the fire marshall to inquire about locking the doors that go out to the street. The facility is requiring all staff to take the Active Supervision TTA training and rewriting their policy on bathroom supervision. Facility has also been in contact with the child's parents about the child continually trying to escape. Children are no longer allowed to use the lobby restroom. | |||
