Three (3) medications were found in Space 2 with no permission to administer the medications.

A safe indoor and outdoor environment was not provided for the children. I observed plant growth coming through the fence line on the large playground. This is a hazadrd to the children as some plants may be poisonous to children. Visibility is limited to seeing any vermin coming through the fence that would be hazardous to childrenon the playground. Cut back the growth on the fence line at least one (1) foot back from fence.

A safe indoor and outdoor environment was not provided for the children. There ia a PVC pipe with an opening at the top where children could possible get their foot or leg caught. The pipe is located on the back playground to the right where the corner of the building and the fence come together.

A child was given over the counter medication without written authorization from a parent to follow the direction of the NC State Health Director during a public health emergency and/or the authorization did not contain the required information. A Child in Space 4 had an over the counter Desitin Cream to adminiter but there was no permission to adminiter form filled out by the parent. Ms. Crouse, director contacted the parent during the visit today, scanned her the permission to adminiter form. The parent completed and scanned the form back to the center during my visit today. Ms. Crouse took the form down to the classroom to file during my visit. This violation was corrected during the visit today.

A safe indoor and outdoor environment was not provided for the children. On the infant playground (furthest to the left when standing on the playground and facing away from the building, three vertical strips that frame the outside windows of the building were observed to be accessible to infants and toddlers and were broken and sharp to the touch. Two plastic gutter covers were observed to be broken, and one of these covers was sharp to the touch. Five push pins were observed to be stuck into the wooden siding of the infant playground, which present a potential safety hazard for infants and toddlers if they were to become loose. Two rusted nails were observed protruding from the wooden base of a set of windows. A metal bracket attached to the gutter closest to the front of the building (facing the parking lot), was observed to be broken and sharp to the touch. On the middle playground, a slim, vertical piece of wooden framing had come detached from the side of the building, and rusted nails were exposed. One plastic gutter cover was observed to be cracked and sharp to the touch. On the third playground used for older Pre-K children, a plastic storage cupboard was observed to have one rusted screw protruding from the bottom right corner. Ms. Crouse removed this screw during the visit. Two vertical strips that frame the outside windows of the building were observed to be accessible to children and were broken and sharp to the touch. A wooden bird house was secured to a metal pole that was observed not to be stable; the pole easily moved back and forth in the ground when I touched it. The gate bordering a small porch was observed to have two rusted eyelets, a rusted hook-shaped hinge, a rusted gate latch, and two rusted gate brackets.
