Screen time was offered to children under three years of age. There were three (3) children, who were two (2) years of age, present during the visit. Upon my arrival, the TV was on and a cartoon was playing.

Bed, cribs, playpens, cots or mats were not placed at least 18" apart or separated by partitions when in use. All six (6) cots were placed in space #1 during the visit. The spaces among cots were between five (5) to nine (9) inches apart.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A bottle of bleach was on the top shelf in the cabinet by the sink in the kitchen area. one (1) can of Lysol was on top of the refrigerator, and one (1) can was on the vanity in the bathroom.
