Noncompliance Area: Foam blocks with teeth marks were on the shelf in the preschool classroom accessible to toddlers who were receiving care in the room and still placing objects in their mouths.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: A shelf in the toddler classroom was covered with a grain covering on the base of the shelf and a white paper-like covering over the top. Appearing to be stuck to the cabinet with adhesive at one time, the covering was peeling in multiple areas.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The approved Child Service Report on file for Child #2 was dated 3/9/2026, more than 6 months from their start date at the facility (see LIS code sheet).

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information).

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings).

Noncompliance Area: Bananas, clementines and apples were on shelf by the kitchen in the preschool classroom and not refrigerated. Staff #1 stated the fruit was not purchased on the day of the inspection.

Correction Required: Fresh fruits and vegetables that are not used on the day of purchase shall be refrigerated.

Noncompliance Area: Based on the dates on each form, the emergency contact form and financial agreement for Child #2 was updated more than 6 months from the prior.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The facility did not have clearances for facility person #2, who cleans the center on Wednesdays while children are in care.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Description: Locked or inaccessible

Noncompliance Area: In the storage room, where children pass through when going outside, Lysol disinfectant wipes were in a bag on the floor, and a small bottle of Gain laundry detergent was on a low shelf. Both were labeled "keep out of the reach of children" and accessible to children. In the bathroom off the toddler classroom, a box of Clorox toilet wand refills labeled "keep out of the reach of children" were accessible in the unlocked cabinet below the sink. The door to the office, which is off the toddler room, was open and 3 Sure Sense air fresheners, labeled "keep out of the reach of children" were in the office stored at a level accessible to children. Off of the preschool classroom, the lock to the right cabinet under the sink in the kitchen was broken allowing Clorox Scentiva, Clorox Cleaner and Bleach and Members Mark Oven, Grill and Fryer Cleaner all labeled "keep out of the reach of children" accessible to children due to the gate to the kitchen not being locked. Lysol disinfecting wipes, labeled "keep out of the reach of children" were accessible to children in an unlocked storage room door in the preschool classroom.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Three cans of shaving cream labeled "keep out of the reach of children" were seen in the storage room off the infant classroom. When asked what they are used for, Staff #1 stated they were used to make slime with the school-age children.

Correction Required: Arts and crafts materials shall be nontoxic.
