| Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 6/24/2025, and are subject to changes pending programmatic review. The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following items were discussed, but not limited to: *Ensure to fluff the wood chips to maintain 6 inches of resilient surfacing as required, *Ensure trash is picked up around the outdoor activity area, *Ensure broom bristles are inaccessible to enrolled children, *Ensure diaper changing containers are covered with a tight-fitting lid, *Ensure diaper products are labeled with the child's first and last name, *Ensure Criminal History Affidavits are signed before the staff's starting date of employment, and *Ensure each activity area has a sufficient quantity of books to meet the needs of the enrolled children in attendance. There were 5 staff files reviewed. 5 of the 5 fingerprint clearance cards were verified to be valid through the DPS website. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business of Rights to the Facility Director at the time of the inspection. | |||
