Findings: The OCC received a complaint from another agency and later received a call from the center to self-report the following: A child was left unsupervised in the bathroom of the classroom for a period of time. The child wandered out of the classroom to the director for help. It is unclear the amount of time the child was left alone. Licensing Specialist was informed that a 4 year-old child was brought to classroom (1) for an afternoon visit without any communication between staff. In addition, another 4 year-old child was sent to the bathroom in classroom (1), never rejoined the group, and the staff were not aware that the child was missing from the group until they were already outside for approximately (5) minutes.

It was approximately (5) minutes between the group going outside and the child being reconnected with the group.

Licensing Specialist was informed that, Hassanat-Tai Olutunde (Aide), asked the 4 year-old child, to go to the bathroom, and forgot that the child was in the bathroom. The children in classroom (1) went outside for their afternoon outdoor time around 4:40 pm, and left the child behind in the classroom bathroom.
