Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: At the time of the Renewal Inspection conducted on 03/27/2025, there were 7 children in the Preschool Classroom. Staff Person #2 identified 3 children in their supervisory group, Staff Person #3 identified 3 children in their supervisory group, leaving child #4 unaccounted for.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. The facility shall have an emergency plan that provides for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.

Noncompliance Area: At the time of the Renewal Inspection conducted on 03/27/2025, the file for Staff Person #4 did not contain verification of the completion of an approved Pediatric First Aid and Cardiopulmonary Resuscitation training from an approved PQAS instructor.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.

Noncompliance Area: At the time of the Renewal Inspection conducted on 03/27/2025, the file for the provisional hire, Staff Person #3, hired 03/10/2025, did not contain the completed clearance request for the DHS FBI clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person #3 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #3 may not work in a child care position at the facility.
