Children were not adequately supervised at all times. On December 19, 2025, on more than one occasion, a staff member is observed on video footage going out of the classroom and leaving eight(8) three-year-old children with no supervision. In addition, on two separate occasions, two staff members are observed scrolling on their cellphones.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On December 19, 2025, a staff member was observed pulling a chair from underneath a three-year-old child as the child attempted to sit down, causing the child to fall to the floor. The staff member then hit the same child on the left side of the face with a piece of bread before placing the bread on the child's plate. Additionally, the staff member sprayed an unknown liquid from a spray bottle on the side of a separate three-year-old child's face and pushed the child in the back.

A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. On December 19, 2025, a staff member is observed hitting three-year-old child with a open hand and pulling the child's hair after the child spilled milk. The staff member also grabbed the child by the back of the neck and forced the child to their seat. On two additional occasions, the staff member pulled the child toward the corner of the classroom, threw the child's shoe towards their body, and kicked the child's chair while the child was seated. Additionally, the staff member grabbed another three-year old child by the upper arms on two occasions. No visible marks or bruises were observed at the time. In addition, the staff grabs another three-year-old forcefully on two different occasions by the upper arms. No marks or bruises were observed. Secondly, the staff thumps a three year old child on their right ear and hits the child in the face which resulted into a lip injury.
