During all this time, staff can be seen searching the facility inside and outside for the child. Around 12:09p.m. the police respond to the Fire Chief's call. The police were able to flag down the staff looking outside for the child. The child was then reunified with the staff and back in the center at 12:13pm. A lack of supervision occurred between the child and staff for approximately 12 minutes. Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group.
