| Findings: During the inspection, a scented plug-in was smelled and observed in the living room (where a child was laying down), in the kitchen eating area and in the bathroom that the children use. The Provider admitted that this has been discussed with her before regarding the prohibition of scented items such as plug-ins and scented tart warmers due to them being a potential hazard for the respiratory systems of children. Provider removed the plug-ins during the last inspection, but they were back in-place during this inspection. Please write a letter of correction stating how you will become in compliance with this regulation in the future. | |||

| Findings: During a review of all emergency forms, it was found that a 5 month old boy is missing all information for his emergency contacts other than their names. A two year old boy's emergency form is missing all of his physician's information. It was also observed that a three year old boy's emergency form is missing his parent's signature and date at the bottom of the form. Please inform the respective parents of the missing information so that they may add it to their respective emergency forms and initial and date the form to indicate the update. | Findings: During the inspection of the outdoor play area, the mesh that was recently installed to prevent entrapment issues has now come away from the playset making it a possible entrapment issue again. | Findings: During the inspection, a 5 month old child was found sleeping in a pack-n-play with a thick blanket on him. These may not be used as rest furnishing for a child who uses a crib or pack-n-play. When this was brought to the attention of the Provider, she stated that the parent brought it for the child. The items listed in this regulation, but not limited to, are potential suffocation hazards and must not be used even if it is provided by the parent. This LS reiterated that the blanket is a potential suffocation hazard and the Provider may want to educate the parent about use of this blanket at home too. The Provider then removed the blanket from that child. Please write a letter of correction stating how you will become in compliance with this regulation in the future. | |||

| Findings: During inspection of the outdoor fenced in area, it was observed that one of the storage buildings, where the children's play area is located, is unsecured. Within the storage building are hazards to include, but not limited to, a large sharp serrated rake, an unsecured chain saw with the blade exposed, a tall ladder that is set up, other tools and multiple chemicals that are located just inside of the door. | Findings: When milk was observed in the refrigerator, only whole milk was present. When asked about milk for the children two years old and older that needs to be 1% or non-fat milk for lunch, the Provider stated that she had milk for them, but used it all this morning at breakfast. The Provider reports being in the Food Program. Today, the Provider's mother is present in the home and is willing to go to the store to pick up 1% or skim milk for today's lunch. While the LS was leaving the FCC home, the Provider's mother arrived with the correct milk for the children two years old and older. Please write a letter of correction stating how you will become in compliance with this regulation in the future. | |||
