| Findings: The health district found the facility out of compliance for supervision, safe sleep practices, and child-staff ratios. An infant was found deceased. During the investigation, it was verified that the child/staff ratios were out of compliance as the provider was caring for more children than allowed. Additionally, safe sleep standards were not practiced as an infant was placed on an insert of a "pack and play" that was on top of a mattress, with a firm pillow on the right side of the infant and a rolled blanket on the left side of the infant. Resolution: During the initial investigation all of the children present were sent home and the provider was sent resources related to safe sleep. Upon completion of the investigation, the facility's license has been revoked by the Department of Health and Welfare. This facility may not provide child care. | |||
