Initial Comments: An Initial Inspection was conducted on 7/16/2024, the following deficiencies were cited and are subject to programmatic review. Senior Compliance Officer- Cara Leyme, MC Compliance Officer Supervisor- Lisa Emery 3 of 3 fingerprint cards were verified on the DPS website to be valid during the inspection. Insurance -valid 7/1/23 until terminated Fire inspection 11/14/2023 Licensure is pending submission of an accepted Plan of Correction. Please submit a POC within 10 days of receiving this report.
