The unlocked outdoor storage closet contained an aerosol can of Hot Shot Wasp and Hornet Killer, a can of Valspar Storm Coat paint, 2 bottles of Zep Intstant Hand Sanitizer Gel, a bottle of Gain Laundry detergent, and a bottle of SAE Motor oil.

Prior to enrollment of children in a Family Child Care Home, and before animals come into the Family Child Care Home, the operator did not obtain each parent's signature on a form acknowledging the type of animal located on the premises and where the animal will be kept during child care operating hours and /or the signed form was not maintained in the child's file. A pet acknowledgement form was not available in the eight children's files reviewed.

Application did not include all required information including, but not limited to: child's full name, named to be called, child's date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. In 3 out of 8 children's files reviewed, the health care needs section was not completed on the child's application.

Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Two packs of diapers, still in the plastic wrapping, were accessible in a child's cubby. In the room used to store child care items, there was a plastic bag on the floor in the closet.
