Noncompliance Area: At the time of the inspection, staff person #3 did not update the health assessment every 24 months, as required. This is evidenced by the current health assessment date of 9/11/25 and the previous health assessment date of 8/26/23.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: At the time of the inspection, staff person #2 did not update the PA State Police clearance, Child Abuse clearance, FBI clearance and NSOR certificate every 60 months, as required. This is evidenced by the current dates of clearances of 11/14/25, 11/21/25, 7/9/25 and 8/18/25, respectively. The previous dates of the clearances were 10/21/20, 10/26/20, 6/27/20 and 3/15/20, respectively. Staff person #2 had direct contact and routine interaction with children.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. The CPSL was revised to include a requirement that all clearances be updated and on file at least every 60 months, prior to the current clearance's expiration.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: At the time of the inspection, there was no signed parental consent for the Levalbuterol that was present at the facility for child #6.

Correction Required: A parent shall provide written consent for administration of medication or a special diet. An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: At the time of the inspection, in the cafeteria used by children in the program, a spray bottle of glass cleaner and a bottle of Virucide was observed on a table and was accessible to children.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Announcement C-24-02 titled " The Handling, Storage, and Disposal of Hazardous Materials and Biological Contaminants" specifies that School-age child care and group programs located in school buildings are to meet these requirements within the areas of the building they occupy. The school-age child care and group programs located in school buildings can provide a written agreement between the school and child care program's legal entity documenting the school's process for the disposal of hazardous materials and biological contaminants if the child care program's facility is unable to meet the above requirement due to the school's daily operations. This document shall remain on file at the child care program's facility.

Noncompliance Area: Staff person #1 has completed mandated reporter training within 90 days of their date of hire (see LIS code sheet), however the mandated reporter training does not meet the minimum of three hours.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to Child Protective Services). Until such time as the required training has been completed, staff person #1 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #1, staff person #1 may not work in a childcare position at the facility.
