Description: Building Surfaces

Noncompliance Area: The inspector observed the ceiling of the indoor play space to be leaking water onto the toys, equipment and flooring, rendering it unsafe for use.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: Surfaces cleaned

Noncompliance Area: An allocated unannounced monitoring was conducted on 9/23/2025. At that time, diapering pad had a small rip in the covering, exposing the foam inside and therefore rendering the surface unable to be properly sanitized after each use.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Description: Emergency plan

Noncompliance Area: An allocated unannounced monitoring was conducted on 9/23/2025. At that time, facility had emergency drill logs dated 4/11/2023 and 4/8/2024, which is past one year.

Correction Required: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility.

Description: Insurance coverage information

Noncompliance Area: Child 1 did not have a health insurance policy number on file.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Child 2 did not have an updated health assessment on file within 60 days of enrollment. Child 2 currently does have an updated health assessment on file, which is dated for 1/27/2025. Please see LIS for date of admission.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Description: ACIP recommended immunization record

Noncompliance Area: Child 1 did not have updated immunizations on file. Please see LIS for date of admission and birth.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: During the renewal inspection on 2/13/23, child #3 was asleep in a crib that did not have 2 feet of space on three sides.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: As of the renewal inspection on 2/13/23, the emergency contact information and the fee agreement for child #1 were last updated on 8/12/22, more than 6 months ago.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: As of the renewal inspection, there was no health assessment on file for child #2 (see code sheet for start date).

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record must contain initial and subsequent health reports.
