| Findings: Supervision Concern. There was a concern of lack of supervision. During the investigation, supervision requirements were not being met during a diaper changing and at the local park. Additionally, the child/staff ratios did not meet standards. Resolution: The provider has moved the diapering station so children can be seen. Provider has also implemented a staff rotation schedule during the afternoon to help with supervision at the park. | |||

At time of investigation, the provider stated there is a handyman on-site when children are not present. At the time of investigation, there was a minor on the premises providing care without adult supervision and without a background check clearance.
