| Findings: Due to ongoing supervision concerns, the health department performed an investigation and determined this facility to be out of compliance with supervision requirements. During the investigation, it was observed that there were two rooms of sleeping children that did not have adult supervision and the provider was unable to have line of sight for both rooms at the same time. The inspector later found the adult in another room assisting other children who do not nap. Resolution: During a follow-up investigation, supervision was in compliance and IdahoSTARS was notified to ensure this provider receives further support with supervision practices. | |||

| Findings: There were supervision concerns that included a child being severely bitten while no staff were actively supervising. During an investigation, it was found that two rooms had sleeping children that were not within sight of staff. Additionally, there was a staff member who is regularly on the premises and does not have the proper background clearance. Resolution: There are new procedures in place for supervising children during quiet time. The provider required staff to take Active Supervision training that was provided by IdahoSTARS. Law enforcement has determined they will not continue an investigation. | |||
