Description: Outdoor equip.- protective surfacing

Noncompliance Area: At the time of the inspection, the multi-colored embedded climber with a fall height of 52.5 inches only had 4" of loose-fill rubber mulch and not the 6" of loose-fill rubber mulch, as required.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair. Tiered LIS: 1. The multi-colored embedded climber in the play yard shall be made inaccessible and will not be used by the children until such time as the loose-fill impact-absorbing material meets the recommendations of the United States Consumer Product Safety Commission. For this piece of equipment, there needs to be at least 6 inches * of loose-fill impact-absorbing material under the equipment. This portion of the plain requires an immediate correction. 2. The operator shall obtain more loose-fill impact absorbing material to bring the depth into compliance with the recommendations of the United States Consumer Product Safety Commission. This portion of the plan requires a date by which this will be complete.

Noncompliance Area: At the time of the inspection, the school age summer camp agreement on file for child #3 and child #6 did not specify the persons designated by a parent to whom the child may be released. The emergency contact form for child #6 did not include the address of the individual designated by the parent to whom the child may be released.

Correction Required: An agreement shall specify the persons designated by a parent to whom the child may be released. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: At the time of the inspection, as acknowledged by staff person #1, copies of incident reports are not being placed in the files of children who have the injuries.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Description: Age and Training

Noncompliance Area: At the time of the inspection, staff person #2, #4, #7, #10 did not take 12 clock hours of childcare training during their respective training period. Staff person #2, staff person #7 and staff person #10 only took 6.5 hours, 4 hours and 6.5 hours of childcare training during their 12/2024-12/2025 training period, respectively. Staff person #4 only took 2 hours of childcare training during the 1/2025-1/2026 training period.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training. Staff person #2, #4, #7, #10 will use hours taken in 2/2026 to fulfill their previous respective training period. Hours used to fulfill the previous training year may not be used to fulfill current training year.

Regulation: 3270.107

Description: Refrigerator

Noncompliance Area: At the time of the inspection, the thermometer in the refrigerator in the Toddler 1 classroom read 52° F.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Description: Review policies and procedures

Noncompliance Area: At the time of the inspection, transportation policies were not included in the parent handbook and the facility was observed transporting children to the pool.

Correction Required: The operator shall review with the parent, at the time of application, the facility's general daily schedule, hours for which care is provided, fees, responsibilities for meals, clothing, health policies, supervision policies, night care policies, dismissal policies, transportation and pick-up arrangements.

Noncompliance Area: At the time of the inspection, the Children's Allergy medication for child #11 was stamped by the manufacturer that the medication expired by 2/2025.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Description: Child's hands washed/Adult Hygiene

Noncompliance Area: At the time of the inspection in the young toddler room, staff person #10 did not wash her hands or the hands of the children after their diaper was changed.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Regulation: 3270.135(b)

Description: Surfaces cleaned

Noncompliance Area: At the time of the inspection in the young toddler room, staff person #10 did not clean the diaper changing surface after each child's diaper was checked or changed.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Noncompliance Area: At the time of the inspection, the health assessment for staff person #7 was dated as being conducted on 12/13/24. This date was after the staff person began working with children (see LIS code sheet). Volunteer #16 did not have a health assessment or TB screening on file and has had direct contact with children.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: At the time of the inspection in the school age room, perishable items including Lunchables, yogurt, and cheese were observed in children's lunch boxes with ice packs and not refrigerated, as required.

Noncompliance Area: At the time of the inspection, the emergency contact form and/or agreement for child #3, #4, #7 and #8 was not updated every 6 months as required. This is evidenced by the current review date of 1/7/25, 1/8/25, 1/7/25 and 1/8/25, respectively. The previous review date was 5/29/24, 6/12/24, 5/24/24, 5/30/24, respectively.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.
