| Findings: Supervision Concerns. There were concerns that child had received a head injury. Additionally, there were concerns of the lack of supervision leading to staff being unable to locate children. The health department performed an investigation and determined that staff had not been performing active supervision allowing another child to scratch a child on the forehead. Resolution: The two staff supervising at the time of the incident will be completing TTA Active Supervision and training on writing incident reports. This incident is considered to be resolved at this time. | |||

| Findings: Supervision Concerns. There were ongoing concerns of a small child coming home with bite marks on a regular basis. Additionally this child fell from their highchair and was taken to a doctor to assess for stitches in their mouth. Upon investigation, the health department determined that there is a substantial supervision concern in this child's classroom. The investigator observed staff not actively supervising children and confirmed that the child fell out of the chair but did not require stitches. Resolution: The provider agreed to keep more experienced aids in the classroom until the new experienced, lead starts. At a follow-up investigation, the health department observed teachers actively supervising the children and the hiring of a new lead teacher with 20 years of child care experience. Additionally, all staff participated in the Active Supervision TTA training conducted by IdahoSTARS. | |||

| Findings: A serious injury was reported by the child care facility. During the investigation, it was confirmed a child slipped and fell on a slide at the park and hit his tooth. Resolution: The child was comforted and a compress was put on the mouth to stop the bleeding. Parents were called and picked the child up. P rovider implemented new schedule changes to stagger class recess times, and required all staff to take Active Supervision TTA Training to better prepare teachers. | |||
